American Family Care (AFC) was established in 1982 in Birmingham, Alabama by Dr. Bruce Irwin. His concept was basic but revolutionary because it put the patient, not the healthcare provider, at the center of the delivery model.

Since 1982, AFC has grown to twenty medical centers across the state of Alabama. In addition to primary care, American Family Care offers Occupational Medicine, General Surgery, Nephrology, Internal Medicine, ENT, Physical Therapy and Prescription services. This "vertical integration" of medical care allows AFC patients access to specialty services, without the traditional two week wait.

Presently, AFC has thirteen facilities in Birmingham, two in Mobile, two in Montgomery and three in Huntsville, Alabama. A typical facility is open 7 days a week.  AFC continues to expand its network of primary care centers to serve more areas of Alabama, while keeping costs affordable.
