Position Summary

The Community of Hope Health Clinic is seeking a full-time Clinic Coordinator to support the efficient operation of our clinical services and ensure patients receive compassionate, organized, and timely care. The Clinic Coordinator plays an important role in coordinating patients, volunteer healthcare providers, staff, and outside healthcare partners. This position is responsible for patient scheduling and communication, clinical preparation, referrals, health screenings, medical record coordination, and other activities necessary for successful clinic operations.
Fluency in both English and Spanish is required.

Preferred Qualifications

  • Bachelor's degree in Public Health, Healthcare Administration, Health Sciences, or a related health field preferred.
  • Must be bilingual and able to communicate effectively in both English and Spanish.
  • Strong organizational, interpersonal, and communication skills.
  • Comfortable interacting professionally and compassionately with patients, staff, healthcare providers, volunteers, and members of the general public.
  • Ability to manage multiple responsibilities and priorities in a clinical environment.
  • Comfortable working with electronic health records and other computer-based systems.
  • Must be available to work some evenings and occasional weekends as clinic operations require.
  • Experience in a healthcare, nonprofit, community health, or patient-services environment is preferred.

Essential Duties and Responsibilities

Clinic Coordination

  • Coordinate assigned clinical activities to promote efficient, high-quality clinic operations.
  • Communicate and coordinate with volunteer healthcare providers regarding schedules, patient needs, and clinic responsibilities.
  • Consult with providers and clinic leadership regarding patient and operational needs.
  • Coordinate patient scheduling, appointment reminders, and other patient communications.
  • Monitor and follow established procedures regarding patient no-shows and cancellations.
  • Coordinate with Volunteer Coordinators regarding provider and volunteer needs.
  • Identify operational concerns and request assistance from the Operations Director or other appropriate staff when needed.

Patient and Medical Record Preparation

  • Prepare patient charts and clinical information before scheduled appointments.
  • Ensure current laboratory and imaging results are available for providers.
  • Maintain appropriate medication information and logs.
  • Ensure patients complete approved screenings as appropriate, including PHQ-9, GAD-7, AD8, and other designated screening tools.
  • Assist with patient health screenings as required.
  • Enter and maintain accurate patient information in the Phoenix EHR system.
  • Prepare physical charts for new patients when required.
  • Obtain appropriate proofs and documentation for patient eligibility.
  • Coordinate annual patient recertification based on the patient's first yearly visit.
  • Request previous clinical records from outside healthcare providers and organizations as needed.

Referrals and Patient Support

  • Coordinate internal and external patient referrals.
  • Assist with mental health and counseling referrals.
  • Communicate with patients regarding referrals, appointments, and follow-up requirements.
  • Coordinate prescription-related communication with internal and external healthcare entities, including new prescriptions, refills, and requests for missing information.
  • Assist patients with Patient Assistance Program applications and required follow-up.

New Patient Coordination

  • Assist with screening new patient applications for eligibility and completeness.
  • Ensure required patient documentation is received and appropriately maintained.
  • Assist new patients in understanding clinic procedures, scheduling, and expectations.

Clinic Readiness

  • Ensure exam rooms, triage areas, restrooms, waiting areas, and other patient-care spaces are clean, organized, stocked, and presentable for clinic operations.
  • Conduct pre-clinic checks to ensure the facility and necessary materials are ready for patients and providers.
  • Monitor clinic areas during operations and address needs as they arise.
  • Complete post-clinic checks and ensure clinical areas are properly prepared for the next clinic session.

Additional Responsibilities

The Clinic Coordinator is expected to work collaboratively with clinic leadership and staff to support the mission of the Community of Hope Health Clinic. The position may perform other duties and responsibilities as assigned by the Executive Director or Operations Director based on the needs of the organization.

Schedule

This is a full-time position averaging 40 hours per week. The Clinic Coordinator must have schedule flexibility and be available to work some evening clinic hours and occasional weekends based on clinic activities and special events.
